Position Description

To link program participants with in-house and broader community resources. Conduct street outreach activities engaging individuals experiencing homelessness.

Part-time position: 20-26 hours per week

SCAN is establishing a project that helps individuals experiencing homelessness and have Severe Mental Illness/Severe Emotional Disturbance within a behavioral health care setting.

Duties: Conduct street outreach activities. Link individuals experiencing homelessness to SCAN programs and other community resources. Provide crisis intervention services. Disseminate care kits (sock, band aids, hygiene products) to individuals in need. Document activities.

Minimum Qualifications:

• High School degree or equivalent
• Associate's degree or equivalent (60 college hours) preferred
• Must be fluent in both Spanish and English
• Good computer skills
• Transporting clients in own vehicle is required at times and compensated for mileage
• Reliable transportation, proof of insurance and a valid driver's license required
• Able to work flexible hours and weekends
