About the Role
Essence Community Care , a portfolio company of Grace Capital , is recruiting for a new initiative aimed at expanding access to high-quality social care services. We are seeking dedicated Bilingual (Spanish) Associate Social Care Specialists to provide comprehensive case management and care coordination services to individuals in need. These roles focus on assessing client needs, connecting them with essential resources, and advocating for improved health and social outcomes. The ideal candidates will have experience working with Medicaid and other government benefits, strong problem-solving skills, and a commitment to serving underserved & vulnerable communities.
Qualifications:
Required
- Bilingual: Spanish , with fluency or advanced-level language skills
- 2+ years of relevant experience with care planning, coordination, and/or management
- 2+ years of demonstrated experience in housing support, government benefit applications, and managing complex cases
- Comfortable conducting outreach and cold calls in medical or community settings
- Must be able to respond sensitively and competently to the service population’s cultural and socio-economic characteristics
- Proven ability to work collaboratively with individuals and groups from diverse backgrounds
- Strong judgment and decision-making skills to advocate effectively for clients’ best interests
- Basic computer skills and comfortable learning new technologies
- Proficient in computer applications, including electronic health record (EHR) systems
- Must have access to a vehicle, proof of insurance and valid driver’s license
- Exceptional interpersonal and communication skills, with the ability to build rapport with individuals from varied professional and personal backgrounds
- Strong time management and critical thinking skills
- Must be able to pass a pre-employment background check, drug screening, and periodic random drug screenings
- The ability to work Saturdays 1-2 times a month or as needed
Preferred
- Bachelor’s degree in Human Services, Social Work, or related field
- CPR/First Aid certification
Responsibilities:
- Manage a caseload of complex cases, ensuring clear communication with patients and caregivers, accurate documentation in EHR systems, and timely data entry, maintenance, and reconciliation
- Provide oversight, guidance, and support to the Social Care team to ensure effective operations
- Connect clients to appropriate community services by offering warm handoffs and providing comprehensive referrals
- Respond promptly to voicemails and emails from participants, community partners, and other stakeholders
- Facilitate seamless navigation across sectors and providers to ensure coordinated service access and completion for all cases
- Conduct pre-screening for client eligibility, assist with applications, disseminate essential information to clients and partner organization staff, and track data for follow-up
- Collaborate with the Social Care Manager to expand outreach efforts and enhance residents’ awareness of available resources and services
- Utilize and maintain up-to-date knowledge of government benefits, policy updates, and program procedures to serve as a trusted resource for clients and colleagues
- Participate actively in internal meetings to stay informed of program updates and operational developments
- Maintain meticulous notes on client interactions and ensure thorough follow-up with referral sources
- Engage in regular meetings with team leads to align on goals, updates, and best practices
- Prepare and submit statistical data and required paperwork in an accurate and timely manner
- Execute additional tasks and responsibilities as assigned by leadership to support the organization’s mission and goals
